Transvaginal Folliculometry (TVF) is an ultrasound examination used for the dynamic monitoring of ovarian follicle development and the ovulation process. The examination allows assessment of the number and size of follicles, monitoring of dominant follicle maturation, and evaluation of the endometrium at different stages of the menstrual cycle.
Folliculometry involves several ultrasound examinations performed at intervals determined by the physician, depending on the length of the menstrual cycle and the purpose of monitoring. The examination is used, among other purposes, to assess and monitor infertility treatment and ovulation stimulation cycles, including assisted reproductive technology (ART) procedures.
Indications
- monitoring the development and maturation of ovarian follicles;
- monitoring ovulation and identifying the probable ovulation period;
- evaluation of cycles with ovulatory disorders;
- suspected anovulation;
- monitoring the menstrual cycle as part of infertility treatment;
- monitoring the ovarian response to ovulation stimulation;
- assessment of the optimal timing for administration of an ovulation-triggering medication, as recommended by the physician;
- monitoring cycles as part of assisted reproductive technology (ART) procedures, including IVF/ICSI;
- dynamic assessment of the endometrium;
- detection and monitoring of ovarian cysts or other ovarian abnormalities;
- other clinical situations, as recommended by the physician.
Procedure
The examination is performed transvaginally using a specialized ultrasound probe. The physician evaluates the ovaries, follicles, and endometrium and performs the necessary measurements. Examinations are repeated at the recommended intervals for dynamic monitoring of changes and assessment of follicular development and ovulation. The results are documented in an ultrasound report.
Limitations

- ultrasound does not always allow ovulation to be definitively confirmed during a single examination;
- in some cases, hormonal tests or other additional examinations may be required;

Contraindications
- the examination may be contraindicated or postponed in cases of acute vaginal infections or lesions, depending on the physician’s assessment;
- severe pain or other situations in which the examination cannot be performed safely and comfortably;
- other individual clinical situations determined by the physician.
